Consumer Rights and Information

In accordance with Texas law (HB 4224) and Section 181.105 of the Texas Health and Safety Code, Texas-licensed behavioral health providers are required to make the following consumer information publicly available. Clients may request access to their health records by contacting our office directly. Requests are processed in accordance with applicable state and federal laws. Our administrative team is available to assist with questions related to the request process. Please contact our office manager with any records requests in writing and send to kwhitburn@bluestarpsychology.com. Fees may be permitted by law.
